Anyone can make a report about a student they are concerned for at ÌÇÐÄvlog¹ÙÍø (including themselves).

Incident Report Form

Used by Faculty and Staff to submit issues involving academic integrity, behavioral concerns, violations to student code of conduct, illness/injury, and general student complaints. Depending on the content area it is routed to either the VP of Student Development or Behavioral Intervention Team. This form can also be found under the faculty and staff tab on the ÌÇÐÄvlog¹ÙÍø Website.

Student Concern Form

Used by students to submit concerns. Concerns are categorized either as a concern about an instructor or class or concern about something else. Instructor/class concerns are routed to the VP of Academic Affairs office and other complaints are routed to the VP of Student Development. Students have the option of submitting their concern anonymously. This form is also found on the ÌÇÐÄvlog¹ÙÍø website under Student Services under the category Forms/Online Tools.

Use this form to submit concerns about student’s health, well-being, and engagement. Concerns regarding a student’s mental health, illness or hospitalization, loss of a loved one, relationship concerns, housing instability or other basic need insecurities, academic engagement, and other non-academic barriers impacting a student’s overall well-being may be reported here.

You may enter a report anonymously via this form. If you choose to do so, please understand it may limit the ability of the Care Team to take action based on the information provided.

This form is for requesting information and support related to the rights of Pregnant Students at ÌÇÐÄvlog¹ÙÍø. This form can be completed by Students, Faculty, or Staff, and will be routed to the Vice President of Student Development or the Access Services office, based on the student’s identified needs.


The reports are completely anonymous and confidential. Based on what ÌÇÐÄvlog¹ÙÍø Cares discovers, struggling students will have the option to receive free counseling. In some cases, no response may be necessary, or an immediate intervention might be called for.

We understand being a college student presents many challenges. Sometimes those challenges can be overwhelming. ÌÇÐÄvlog¹ÙÍø Cares works with students who need assistance with issues ranging from class absenteeism to threats of self-harm or harm to others. Together, we can help keep ÌÇÐÄvlog¹ÙÍø students safe.
